Clinical Pharmacist @ Fortis

Job Description

• Reporting of Medication errors and Adverse Drug Reactions to MS/Quality-Head / and the root cause analysis of errors
• Drug allergies, Drug-Drug interactions and therapeutic duplication
• Completeness, correctness of prescriptions.
• Proper drug segregation, labeling and stacking of drugs in sub stores
• Ensure proper drug storage in Pharmacy
• Ensure Narcotics and High risk drugs are stored & maintained properly
• Crash cart maintenance
• Ambulance maintenance
• Data collection regarding Antibiotic usage and its calculation based on organizational/ accreditation requirements.
• Training to the Nurses, Pharmacists and Medical Transcriptionists on

1. Nurses-About Dilution practices, Administration Procedures, documentation & storage of drugs with proper label and policies on MOM to reduce the Administration errors and Documentation errors.

2. Storage of Drugs and proper Dispensing procedures and policies on MOM for reducing Dispensing errors.

3. Medical Transcriptionist- To transcribe drug orders properly by reducing transcription errors and indenting errors and about policies on MOM.

• Preparation of Monthly Audit Report on MOM
• Updating of Drug Formulary
• Medication Audit to ensure adherence of policies in all areas of hospital
• Check Nurses Administration procedures of drugs
• Initiation of Projects for the improvement of Quality of the Institute
