Job Description

Role & responsibilities


  • Assemble, organize, and maintain patient medical records (paper and electronic)
  • Review records for completeness, accuracy, and compliance
  • Enter, update, and retrieve patient information in Electronic Health Record (EHR) systems
  • Ensure confidentiality and compliance with ART regulations
  • Process requests for medical records from patients, providers, and insurance companies
  • Scan, index, and file medical documents
  • Assist with audits and quality control reviews
  • Coordinate with clinical staff to resolve documentation deficiencies
  • Track record locations and maintain record retention schedules
