Role & responsibilities

Review, analyze, and assign accurate ICD-10-CM codes for HCC (Hierarchical Condition Categories) based on medical records and documentation.

Ensure all diagnoses are captured and coded as per CMS guidelines and client specifications.

Validate documentation for accuracy and completeness to support the assigned codes.

Work on risk adjustment models and identify opportunities to improve coding quality and accuracy.

Maintain a minimum of 95% coding accuracy and daily productivity targets.

Stay updated with the latest coding updates, compliance standards, and payer requirements.

Collaborate with the Quality and Audit teams to address feedback and ensure continuous improvement.

Adhere to HIPAA regulations and maintain strict confidentiality of patient information.

Participate in coding refresher sessions and team meetings to enhance domain knowledge.

Preferred candidate profile

CPC / CRC certification (preferred 2 to 4 years' experience as hcc coder
